- ECG is insensitive
- ECG criteria require QRS duration less than 0.12 s
- Right axis deviation of +110° or more
- Dominant R wave in lead V1 (R/S ratio > 1)
- R wave in lead V1 7 mm
- Supporting ECG criteria
- ST segment depression and T wave inversion in leads V1 to V4
- Deep S waves in leads V5, V6, I, and aVL
- Other causes of tall R wave in V1
- Posterior myocardial infarction
- Type A Wolff-Parkinson-White syndrome
