Systolic murmur location

Aortic stenosis

• Typically loudest at second right intercostal space.
• Radiates to right carotid artery and right clavicle.

Mitral regurgitation

• Loudest at left fifth or sixth intercostal space in mid thorax.
• Radiates to left anterior axillary line or into axilla.

Tricuspid regurgitation

• Loudest at lower left sternal border.
• Radiates to right lower sternal border and epigastrium.

Hypertrophic obstructive cardiomyopathy

• Loudest at fifth intercostal space in mid left thorax.
• Radiates to lower left sternal border.
