Systolic murmur location
Aortic stenosis
- Typically loudest at second right intercostal space.
- Radiates to right carotid artery and right clavicle.
Mitral regurgitation
- Loudest at left fifth or sixth intercostal space in mid thorax.
- Radiates to left anterior axillary line or into axilla.
Tricuspid regurgitation
- Loudest at lower left sternal border.
- Radiates to right lower sternal border and epigastrium.
Hypertrophic obstructive cardiomyopathy
- Loudest at fifth intercostal space in mid left thorax.
- Radiates to lower left sternal border.
