This clinic is a caring, patient-centered healthcare center that provides excellent medical and dental care. We are a community health center with medical clinic, dental clinic, behavioral health, lab, and in-house pharmacy. We utilize dental integration in the medical clinic which highlights total wellness as an education point to patients. This full time position provides diagnostic, therapeutic, and emergency dental services to patients, and will also provide preventive dental services and education on oral and dental hygiene. The dentist contributes in the development of dental specific quality measures and goals for the organization. The dentist also assures integration of patient education into all phases of the treatment plan. The organization fosters a great learning environment with a good mix of faculty and resident physicians. Over the past 5 years, with the guidance from TransforMed, the Center has made great strides moving forward with the patient-centered medical home concept. Recently, the Center has started having substance abuse group visits on site. Additionally, the launching of a web-portal has expanded patient access to individual medical records and their providers. In 2008, the Center began using an electronic medical record system, Practice Partners, which has helped decrease the time required for documenting office visits and has increased the number of patients seen per day. We are also proud to host a Family Medicine Residency Program, and our medical clinic is currently under expansion to add more exam rooms. Position schedule is M-Th: 8a-6p, F: 8a-12p. Salary is $118,560-124,800 and includes performance based salary increases and bonus opportunities. This is on a 1 year contract. Additionally, Loan Repayment, Relocation Costs, and CME Allowances are available. The Center accepts most insurance: Private Insurance, Medicaid, and Medicare. For patients without insurance, the center offers an income-based discount program for those who meet the qualification guidelines. We are a FQHC with a medical clinic (newborn to end-of-life care, dental clinic (preventative, diagnostic, restorative and ECP outreach care), family medicine residency program, in-house behavioral health and substance abuse counseling, and an in-house pharmacy. We have roughly 120 employees at our facility who keep the mission of providing health care to our patients.

Community Description:
The community serves approximately 50k people, but yet still has an intimate, "small-town" charm. It is an economically strong, culturally diverse place where there are plenty of things to do and enjoy, yet still safe and stable enough to raise a family or grow meaningful relationships. Our facility is only a few miles from a regional hospital. We have a great community, and are very excited about the providers who will join our team, and benefit the greater community as a whole.
