The University of Kansas Medical Center participates in annual reporting
of conflicts of interest in order to comply with Federal regulations
and Kansas Board of Regents’policy. Federal regulations require
100% compliance with conflict of interest reporting.
Financial relationships between sponsors and researchers are increasingly in
the public eye so this process is also a critical step in maintaining the integrity
of our research programs. Conflict of interest reporting contributes to both
human subject safety and research integrity. It protects the reputation of the
researcher, as well as that of the University, while providing research subjects
pertinent information for giving an informed consent.
Annual reporting is required of
Principal investigators with KUMC appointments must ensure that all staff (including KUPI, KUHA or VA employees), students and residents participating in their research complete this form.
Conflict of interest reporting is confidential and will be handled as such. When real or perceived conflicts of interest are identified, the goal is to develop a management plan with the Conflict of Interest Committee so that subject safety is not compromised and the research is not impeded.
To complete the required reporting:
The Conflict of Interest Disclosure Form for 2008 fiscal year is available for immediate completion in the CHALK training site. Conflict of Interest (COI) reporting is required of all University of Kansas Medical Center and KU School of Medicine-Wichita faculty and unclassified staff, as well as all individuals who are study personnel in KUMC or KUSM-W research.
PLEASE NOTE: AS OF JULY 2007, CONFLICT OF INTEREST REPORTING HAS CHANGED.
This year for the first time, some users may be permitted to reconfirm their previous year's statement without being required to complete a new form. The two reporting methods are described below:
An Abbreviated Reporting Option is available if 1) You have electronically submitted a Conflict of Interest disclosure through Chalk since July 1, 2006; 2) You were NOT required to submit a Form B at that time; and 3) There have been no changes in your disclosure information since your last report.
Federal regulations are requiring 100% compliance with conflict of interest reporting. Conflict of interest reporting is confidential and will be handled as such.
Please complete the required disclosure by Monday, September 17, 2007.

To access the Disclosure form:
1. Go to the CHALK login page at http://www2.kumc.edu/chalk2/.
2. Choose University of Kansas Medical Center for your affiliation.
3. Enter your KUMC network account username and password. This is the same information you use to login to the network when you turn on your computer.
4. You will be taken to your personalized training record. Here you will see a list of the online training courses available to you in CHALK. Select the option "Conflict of Interest Disclosure.”
5. After reading the “Conflict of Interest 2008 Fiscal Year Disclosure Instructions,” click on the gray button at the bottom of the screen to begin a new Conflict of Interest Form.
If you are required to complete a Conflict of Interest Disclosure but do not have a KUMC network account, complete a guest access request at: www2.kumc.edu/compliance/chalkcourtesyform.html.
