Checklist for PRIDE Administration.

  1. The resident in the community pediatrics rotation will pick up their checklist by the fifth of the month. If they have not, please contact the designated resident.
  2. A letter will be brought in by the resident by the 15th. Linda Schuck and either Randy Goldstein or Lisa Gilmer should proofread the letter. It should be typed on PRIDE letterhead.
  3. Three photocopies of this letter should be made. The original is to be paper-clipped to the Business Support letter and the FOX 4 Information letter. This packet will be picked up by the resident for presentation to the winner. The first copy will be given to the principal during the presentation. The second copy is kept in Randy Goldstein’s file, and the final copy is for Dr. Lindsley’s files.
  4. Call the principal of the school and announce the name of the child that has been chosen. Inform the principal that we will present the award to the child in the class or during a school assembly on the last Thursday of the month, preferably around 2 or 2:30 PM. The presentation normally lasts about 10-15 minutes. The front parking area (usually a circle drive adjacent to the school) should be open for our cars to park during that time. The principal is invited to attend the presentation.
    The parents should be called by the principal and invited to attend. The parents should call Linda Schuck at KU to get information about the FOX 4 Morning Show the following Friday. Tell the principal that the child probably will be late on Friday due to this interview.
  5. If a limousine is involved with the school assembly, inform the principal that a standard "field trip" release must be signed by the parents to allow us to take the child and parents home from school after the presentation.
  6. If Blades tickets are involved with the award fax a Message Board Announcement to 842-6673 to insure the child’s name is announced during the game.
  7. A Media Advisory should be sent to Bob Hallinan’s office (fax-588-1225).
  8. The Medical Alumni Office must be called to reserve the JayDoc suit by calling extension 1255. We will pick the suit up around 1:30 and return it by 4:30. The resident is responsible for the pick-up and return.
  9. A certificate should be printed out and sent with the packet. This is for the principal to post in the school.
  10. The resident is responsible to take a shirt for the child, the letter and packet for the child, the letter and certificate for the principal, and the prize.
