Any murmur associated with symptoms or signs of congestive heart failure,
exertional angina, or syncope should be considered organic.

The following murmurs should be considered organic even in
an asymptomatic patient.

  1. Regurgitant systolic murmurs (Ventricular Septal Defect audio example).



  2. VSD video example.



  3. Murmurs associated with: a) thrill, b) a click, c) and abnormal S2, d) an abnormal precordium, e) abnormal pulses, f) S4.

  4. Systolic murmurs which increase in intensity upon standing, with Valsalva.

  5. All diastolic murmurs.

  6. Continuous murmurs anywhere in the chest.

  7. Continuous bruit over heart and/or liver (common sites of large A-V malformations).
