Drug Policy

 

Intravenous Immune Globulin (IVIG) Policy

 

In anticipation of a national drug shortage, the Pharmacy and Therapeutics Committee approved a new policy addressing indications of use for intravenous immune globulin (IVIG).  This policy was developed by a multi-disciplinary committee with representation from Hematology, Neurology, Infectious Diseases, Neonatology, Pharmacy and others.

 

All other uses of IVIG outside of the policy’s indications will require secondary review and approval by a member of the Medication Use and Policy Management Subcommittee, after consultation with the prescribing physician.

 

 

Protocol for use of Optison in Echocardiography

The Pharmacy and Therapeutics Committee approved a protocol for the use of either Optison or agitated saline in Echocardiography, in order to improve safety and standardize care.  The new order set for echocardiography procedures (ECHO) will require a physician’s signature.  These agents will now be stored on the floors for convenience and increased accessibility.

Withdrawal of Hydromorphone Extended Release (Palladone)

 

On July 13, 2005, the Food and Drug Administration (FDA) requested the withdrawal of extended release hydromorphone capsules (Palladone, Purdue Pharma) due to safety concerns.  Palladone was marketed as a once-daily formulation of hydromorphone indicated for use in opioid-tolerant patients experiencing persistent, moderate-to-severe chronic pain. 

 

Recent information provided by Purdue Pharma showed a serious and potentially fatal dose-dumping effect when taken with alcohol, secondary to a compromise of the extended release mechanism of the capsule.  Dose-dumping refers to the rapid release of hydromorphone from the extended release product into the bloodstream.  All sales and marketing of Palladone in the Untied States have been suspended.
