The Stroke Impact Scale is a copyrighted instrument. It can be copied and used freely but

YOU MUST HAVE MICROSOFT WORD AND MICROSOFT ACCESS (OFFICE 97 OR HIGHER) TO READ THE DOCUMENTS ON THIS PAGE AND USE THE SIS DATABASE.

There are 4 steps to acquire the Stroke Impact Scale or SIS16 Database:

  1. Print the attachment below called "SIS agreement - Non-Profit" (3 pages). Read this document and if you are willing to sign this agreement, mail as instructed, and abide by the guidelines, proceed to step two. If you work for or represent a for-profit organization, you will need to contact Sue Min Lai, PhD for more information about registering your use of the Stroke Impact Scale.

  2. Print instructions for downloading, setting up, and using the SIS database. Click on the "Instructions for Downloading SIS Database" below and print.

  3. Print the "SIS Guide for Administration" (2 pages)

  4. Click on "Download SIS Database" or "Download SIS16 Database" below. This will download a self-extracting zip file that will unzip to create a SIS database (ACCESS) for your use on your PC.
